Compressed Oral Tablets
Compressed tablets are one of the simplest and easiest ways for patients to take their medications. We compound them with one or multiple medication ingredients, in a variety of combinations. In addition to the Active Pharmaceutical Ingredients (APIs), tablets may be compounded with binders to keep the tablet together, lubricants to make them easier to swallow, and flavorants to make them more palatable to patients. Tablet-delivered medications enter the bloodstream from the stomach or bowel.

Fast-burst Sublingual Tablets
Our fast-burst sublingual tablets are scored tablets that dissolve quickly under your tongue. The active pharmaceutical ingredients are designed to be absorbed through the mucosa in the mouth delivering the drug into the bloodstream.

Belmar sublingual tablets have several advantages over troches. Troches use polyethylene glycol as their base, which may cause irritation to patients’ gums. More and more patients are trying to avoid polyethylene glycol due to sensitivities. Also, unlike troches, sublingual tablets do not melt if they get hot and there are no special shipping requirements.

Vaginal tablets and vaginal suppositories can provide fast, targeted relief from localized conditions affecting the vaginal tissue. These solid-form medications are inserted into the vagina, where they melt as they reach body temperature and are designed to absorb into the tissues and bloodstream. Vaginal tablets and suppositories can address vaginal infections or relieve vaginal irritation and dryness due to menopause. Vaginal suppositories and tablets can deliver other medications, such as diazepam to relax pelvic muscle spasms often present in conditions like vaginismus.
Vaginal tablet and suppository advantages:
- Avoid first-pass metabolism
- Localized absorption
- Avoid nausea and vomiting that can be caused by gastric irritation
- Requires a lower dose to reach the site of action, reducing potential systemic side effects
Our vaginal tablets are pH balanced. Maintaining the normal pH balance of the vagina is important to vaginal health since its normal acidity helps fight off infections.
Rectal suppositories are also available through Belmar.

Hormone Pellets

Hormone Pellets
Hormone pellets are a unique delivery form created to replicate how hormones are naturally released in the body as closely as possible. In a simple, quick office procedure, these rice-grain-sized pellets are inserted under a patient’s skin near the hip area via a trocar.
Delivering hormone replacement therapy in pellet form has multiple benefits:
- Closest to nature — Pellets mimic the body’s natural delivery system more closely than any other method.
- Consistency — Pellets supply a consistent level of hormones in the bloodstream.
- Convenience — Hormone pellets only need to be inserted 2-4 times per year, potentially replacing oral medications that are often dosed to take daily.
- Fewer side effects — Pellet therapy bypasses the liver, sending hormones directly into the bloodstream, and avoiding certain side effects more likely to occur with oral dosage forms.
Some patients report feeling significant symptom benefits in as little as four weeks, although maximum benefits may take up to six months. It’s important to note that pellet therapy is not new, but rather, physicians in the United States have been using it since the late 1930s.
Pure Pellets vs. Stearic Acid Pellets
It’s important to note that the ingredients in pellets make a difference in the overall experience for patients. At Belmar, we do not use stearic acid as a filler in our compounded pellets.
Stearic acid is an inactive, wax-like fatty acid that is not water-soluble. When used in pharmaceuticals as an emulsifying agent or binder, stearic acid can be very irritating, sometimes leading to an allergic reaction. Pellets internally bound with stearic acid can have an increased risk of extrusions, infections, and fibrosis.
Pure pellets, like the ones we compound at Belmar Pharmacy, are 100% hormones, with just trace amounts of stearic acid on the outside of the pellet. This trace amount acts only as a lubricant in production and is clinically negligible at less than 0.1%.
What About Cholesterol Pellets?
Cholesterol pellets were created from a demand by prescribers requesting pellets without stearic acid. Since cholesterol is a naturally occurring substance, the body tends to recognize it when it’s used as a binder in pellets. As a result, many providers advocate that cholesterol pellets can produce less side effects, such as inflammation, infections, fibrosis, and extrusions.

Pellet Ingredients Matter
When it comes to bioidentical hormone replacement therapy (BHRT), pellets are often the preferred option for many women and men. However, the quality of the pellet is extremely important, and not all pellets are created equally.

Transdermal Compound Creams
Belmar compounds steroid hormones for transdermal absorption in an exclusive cream base. Our cream base contains all-natural ingredients and has no glycols, parabens, or petroleum products. Our creams can be dispensed in jars with measuring spoons, tubes, Topi-Click®, or syringes. The high water content ensures absorption through the microcirculation system and minimizes the chance of transferring any of the hormones by contact.
Cream base advantages:
- Very high water to lipid ratio means faster absorption. Small, lipid-soluble steroid hormones leave the base ready for the lipid stratum corneum (water likes water and fat likes fat)
- Less time spent on the skin helps avoid transferral to loved ones or pets
- Petroleum- and paraben-free, non-comedogenic
- Luxurious feel, can be used alone as a face and skin moisturizer
Vaginal Creams
Belmar compounds a variety of vaginal creams to treat atrophic vaginitis, vulvodynia, and other chronic vulvovaginal disorders that cause discomfort or pain and disruption of healthy sexual activity.
Vaginal cream advantages:
- Lower doses of medication can be used when applied directly to the site of activity
- Circulating blood levels of hormones aren’t raised significantly, so breast and endometrial tissues are less exposed
Topical Gels
A compounded gel is a solid or semisolid system of at least two constituents — a condensed mass and an interpenetrating liquid. They are applied to the skin for slow and prolonged absorption and can be an effective and targeted therapy for skin conditions.
Topical gel advantages:
- Less greasy than some other bases and can be easily removed from the skin
- Localized effects with minimum side effects, because systemic circulation is largely avoided

An ointment is a preparation for topical use that contains an oil base. Ointments have a higher concentration of oil than creams, which have a higher concentration of water. There are different kinds of ointments; they can be single phase, hydrophilic or hydrophobic and must be matched to the type of medication being delivered.
