Setting up an account
To get started, all new clients must complete our account application, which includes important disclosures and agreements. These documents verify your practice’s eligibility to purchase compounded medications.
Preparing for your application
- General contact and billing details
- Prescribing practitioner name, state license, DEA registration, and NPI
- Authorized agents
- Strengths, types, and quantities of medications desired
- Collaborative or practice agreement
What are Authorized Agent forms?
An authorized agent is the individual permitted to:- Place orders on your behalf

What are DEA disclosures?
To comply with federal regulations, we require a DEA disclosure confirming that your practice is legally authorized to handle controlled substances. This is a critical safeguard for:- Legal compliance
- Order traceability
- Protecting your practice and our manufacturing facility
Payment terms and methods
Before orders can be shipped, payment must be received. Here’s what to expect:
- Payment Terms: Due at time of order processing
- Accepted Payment Method: Credit card only
- Additional Agreements: You will sign an attestation page during your account application and agree to terms when placing orders.
Placing an order
All orders are placed and managed through our LifeFile online portal. Once your account is active:
Orders can be placed anytime through the portal or by calling 877-267-3410
All required documentation is submitted during the initial application process
Turnaround time: Most orders ship within 24 hours
Receiving your order
When receiving your shipment:
- You must confirm you hold an active DEA license and state license
- Agree to all terms and conditions outlined during the ordering process
- Please review your packing slip and verify the package contains the correct dose and quantities of medications
If there is a discrepancy in your order, contact us immediately at

How to report quality concerns or adverse events
Maintaining quality and safety is our top priority. If you experience a quality issue with a product OR if a patient develops a serious adverse event, it should be reported to Belmar Select Outsourcing 877-267-3410 AND the Food and Drug Administration www.fda.gov/medwatch and 800-FDA-1088
Do not dispose of the product. We may require a return for investigation.
What is the shelf life of pellets from Belmar Select Outsourcing?
Pellets from Belmar Select Outsourcing have up to a 360-day shelf life from the date of compounding. We do not ship any product with less than 30 days of shelf life remaining without your approval. For large orders, call our Support Team to confirm expiration dates in advance. Expired pellets cannot be returned, exchanged, or replaced.
